ATTENTION SPORTS FANS !!!!

Fall 2008 Boys and Girls Soccer tryouts will be the week of September 22nd.

Sports forms are due by September 10th.

Please be aware sports information will be announced in gym class,

during school announcements on ETV and on our web site.

It is the athletes responsibility to look out for these postings, speak with the coaches and hand in the completed forms by the deadline date.

THE MEDICAL ELIGIBILITY CHECKLIST FORM FOR THE OVERVIEW OF REQUIREMENTS  

Helpful Tips:               

*** All students need to have the Part A Health History Questionnaire completed for each sport season. At the time of the physical exam the Health History Questionnaire needs to be reviewed and signed by your health care provider along with your signature. If your child is trying out for another sport later on in the school year, another Health History Questionnaire needs to be completed and signed by a parent only. 

*** All students trying out for sports will have to present written proof of a Physical Examination. At the time of the physical exam, the Part A Health History Questionnaire needs to be reviewed and signed my your health care provider.

*** The physician must complete the vision with or without correction, blood pressure, pulse, height and weight.

*** All forms are to be handed in person to the school nurse.

*** Only forms for the announced sports will be accepted.

*** Please review the forms before submitting them into the Nurses office.

*** These forms must also be cosigned by our District doctor prior to the start of the practice season tryouts. If they are incomplete the District doctor will not approve the forms and they will have to be returned to you for completion. This may delay the student's clearance to play.
