Important News for Parents and Student Athletes!
All students who wish to participate in Eisenhower Middle Schools sports are required to complete our
Sports Participation Forms which includes having a physical examination by a licensed health care provider. These forms should be downloaded from our web site. The forms are posted on the Nurse's Office web page. From the Nurse's web page there is a checklist to help you complete the process. Students must establish eligibility for each season. Only sports forms for the announced sport/season will be accepted. Please be aware sports information will be announced in gym class, during school announcements on ETV and on our web site. It is the athletes responsibility to look out for these postings, speak with the coaches and hand in the completed forms by the deadline dates.
